Physician and Medical Director:
Directs patient plan of care and oversees the pain and symptom management.

Hospice Registered Nurse:
Coordinates the care provided by the hospice team. Our nurse completes the initial assessment and plan of care which is approved by your physicians and our Medical Director.

Pharmacist:
Reviews medications and effectiveness with the physician and hospice nurse. Our pharmacist focuses on medications that provide pain relief, comfort and symptom management.

Certified Nursing Assistant:
Provides personal care, including bathing, grooming and skin care. Works with the nurse and family to determine frequency of care.

Social Worker:
Provides emotional and psychological support to the patient and family. Also helps with financial planning and accessing appropriate community resources.

Spiritual Care Coordinator:
Works with patient and family clergy and provides spiritual care as requested by the family. Our spiritual care coordinator is available for prayer and counseling to help the patient and family cope.

Volunteer Coordinator:
Recruits, trains and directs volunteers to assist the patient and family with shopping, household tasks and other needs.

Bereavement Coordinator:
Provides emotional support to the family for up to 13 months-offering counseling, education, and memorial services.
