Referrals:
Please call 1-888-682-9996 to make a referral 24 hours-a-day, seven days-a-week. Our intake coordinator will work with you to ensure a prompt evaluation and admission to Hospice Services of Alabama based on the needs of the patient, family and referral source.
Needed Materials:
Please fax as much of the following as possible to (205) 682-9994
1) Physicians Order
2) Face Sheet (history and physical if possible)
3) Equipment needs (oxygen, hospital bed, etc.)
4) Prescriptions (if known)
Note:
If needed, a member or our staff will come to your facility and copy the necessary records provided there is a physician order for hospice care. In addition we are available for patient/family education at any time in person or through our Hospice Helpline toll free at 888-682-9996.
